Chronic diseases, their management and associated financial burden are among the major challenges for health care systems today.
For patients and their families, managing chronic diseases is vital for determining their quality of life and how confident and accepting health care institutions are towards them.
This is why Keralty has developed an integrated model to approach chronic health conditions. This model is sensible, humane and provides care with high scientific standards, manages access to services required in a timely and coordinated manner, and suits its operations to the special daily living and vulnerability conditions faced by people with advanced chronic diseases.
When we apply this model widely, focusing particularly on people who have developed multiple chronic conditions and are more fragile than others, results show that we have achieved not only the health indicators' targets and intended economic goals, but have also made a difference by bringing satisfaction and well-being to a population deserving of our highest consideration and support.

Palliative care program
Our Organization, committed to people's health from birth to death, decided to develop a comprehensive palliative care program, as one of the main pillars of its mission and vision of service.
The program enhances the individuals' dignity, and is based on compassion and understanding of human pain, reflecting our commitment to providing services in an efficient manner, while seeking to:
- Take care of patients in a holistic manner, by analyzing, understanding and taking care of their illnesses and all conditions and consequent complications.
- Identify the symptoms that bring discomfort to the patient at an early stage, and control them.
- Accompany patients and their families medically, socially and spiritually during the process of illness and grief, helping them understand that death is a part of life.
- Help patients stay as active as possible until their time of death.
- Improve the potential quality of life for patients and their families.
- Reduce the underlying risk of readmission or re-consultation to emergency and/or hospitalization services.

In the context of Keralty's commitment to designing and implementing holistic and integrated health care models (from insurance to health care delivery), by combining the principles of primary care, the 5 P's, shared value, and community development, at the service of isolated and disadvantaged populations, isolated for different reasons (actual limitations to their access to the health care system, geographic, catastrophes, conflicts, or unstructured), and from our vision of health as a vital vector of economic, even social development, we have been collaborating with several governments and companies to create new health care models, under the following intervention conditions:
- Focus on primary care based on intervention and health care delivery services through comprehensive care centers networks.
- Portfolio of health care delivery services focused on critical needs and demands, after an ad hoc health risk characterization, which will be managed based on its outcomes.
- Assigned populations in specific long-term frameworks.
- Management of referral and counter-referral with other required levels.
- Public-private (Keralty with different institutional and government levels) and private-private partnerships with companies and private entities with relevant presence in the communities in which we operate, facilitating processes of care, provision of assets, infrastructure and complementary health care support.
- Convergent action from KERALTY GROUP (Group's companies, Sanitas University Foundation, Sanitas Foundation).
